Breast Augmentation Quick Facts

Breast augmentation (breast implant surgery) is the number one cosmetic surgery procedure performed in the United States.  Brian S. Procedure: Breast Augmentation

Best Candidates: Women who want to enhance the size of their breasts, are in good health, have reasonable expectations and whom are not pregnant or nursing.

Inpatient/Outpatient: Outpatient surgery

Where:  Dr. Glatt performs all of his breast augmentation surgeries at either Morristown Memorial Hospital, Morristown Memorial Hospital outpatient center, Saint Barnabas Medical Center or Saint Barnabas Ambulatory Care Center (ACC)

Anesthesia for Breast Augmentation: General anesthesia, which puts the patient completely to sleep.  This is necessary so the muscle can be fully “relaxed” for the implant to be easily placed beneath it. 

Dressings and After Care for Breast Augmentation:  Dr. Glatt’s patients go home wrapped in a stretchy band (tube top) that needs to remain on until the first post op visit 2 days later.  At this visit the patient will bring a few different types and size sports bras, this sports bra must be worn as much as possible for 3 weeks.

Recovery Time of Breast Augmentation: After surgery most patients experience fatigue, swelling, and only moderate discomfort.  Most patients are driving within 6-7 days after their breast augmentation surgery.  It is extremely important not to perform strenuous activities for the first 3 weeks while you are recovering from surgery.  After that, you can gradually increase your activity level. 

Medication after Breast Augmentation:  Dr. Glatt provides his patients with all of their prescriptions prior their surgery so they readily available once they arrive home.   Typically most patients require the use of pain medication for 2-3 days.  Then a muscle relaxant such as Valium may be used for another 2-3 days which helps with muscle spasms. You will also be provided with an antibiotic to guard against infection.

Morristown Mommy Makeover

Mommy Makeover in time for the summer?  Definitely!!

MORRISTOWN, NEW JERSEY – Dr. Brian S Glatt, MD, FACS of the Premier Plastic Surgery Center of New Jersey in Morristown, NJ says “now is the time to start thinking about getting that body in shape for the summer.  There are so many surgical options available to help improve the contour of the breast, abdominal, and thigh regions, many which can be combined at the same time.”  Performing multiple procedures at once has the advantage of reducing cost as well as reducing overall recovery time.  Safety must always be the top priority, however, and typically you would not want to exceed 6-7 hours of surgery at one setting.

The “mommy makeover” is ideally suited to revitalize the abdomen and breast area all at once, within as safe time frame from a surgical standpoint.  By far the most common procedure performed as part of a mommy makeover is a tummy tuck (abdominoplasty).  During this procedure, excess abdominal skin and soft tissue is removed and the stretched out abdominal wall (containing the muscles) is tightened back up internally like a corset.  Liposuction is frequently performed at the same time for additional contouring and sculpting, especially of the sides (flanks) and pubic region to smooth out these areas to match the newly flattened out tummy.  Very frequently, breast implant surgery (breast augmentation) may be desired at the same time as an abdominoplasty to increase the size the breasts and provide volume.  Silicone gel implants have become a very popular option to provide a natural and smooth augmented breast contour, however saline  breast implants are still frequently used as well.  If the breast tissue is deflated or the skin stretched out and loose, then a breast lift (mastopexy) may need to be performed also.  Breast lift surgery tightens up the outer skin envelope, rounds out the contour of the breast and lifts the nipple area back to where it needs to be.  Combining a tummy tuck (abdominoplasty), breast lift (mastopexy), breast implants (breast augmentation), and liposuction remains the essence of a mommy makeover and can result in a dramatic change in the breast and abdominal region.

A mommy makeover surgery can take anywhere from 3-7 hours and does require some sort of anesthesia, usually general anesthesia.  Surgery should only be performed  at a certified operating room by a board certified plastic surgeon, such as Brian S. Glatt MD, FACS, who has a great deal of experience performing this type of surgery.  Recovery time varies, but it generally takes 3-6 weeks until you are back to your routine after this kind of surgery, and recovery details are a  large part of your preoperative discussion or consultation.

Stretch Marks After Pregnancy

As a board certified plastic surgeon who sees a great number of young moms seeking to rejuvenate their bodies after childbirth, the issue of stretch marks comes up quite often.  The key to managing stretch marks is all about prevention.  I have many patients who wound up without a single stretch mark that swear it was because they lathered up their skin with cocoa butter twice a day every day of their pregnancy.  With that being said, there is definitely a genetic basis regarding who will develop stretch marks (and how bad they will be) and who won’t.  Some women are just lucky and based on their physical make-up, will never develop one stretch mark – whether they use products for prevention or not.  Others will use everything on the shelf imaginable, yet still be left with severe stretch marks on their abdomen, sides (flanks), and even their breasts.  Even though there is some genetic basis for this, it is thought that stretch marks can at least be minimized with regular moisturizing treatment of the skin.  This is especially true for women who are carrying multiples (e.g. twins or triplets) and for those women who gain a lot of weight very quickly.  Gaining weight more gradually may help in prevention of stretch marks, as will consumption of foods with high content of vitamins A, C, D, E, zinc, omega 3, essential fatty acids and protein.  This will help your skin produce more elasticity and aid in extra healthy collagen production.  In addition, staying adequately hydrated during pregnancy is thought to enhance the moisture content of the skin and may also contribute to the prevention of stretch mark formation. 

 

Some say stretch marks are somewhat preventable, and this is probably partly true.  Regular use of a moisturizing product during pregnancy is thought to help keep stretch marks away or at least at bay.  One of the most popular of these treatments by far, and the one I favor the most, is straight forward cocoa butter.  My favorite is Palmer’s brand, and you want to stay with the pure cocoa butter stick or cocoa butter in the tub – not the creams with cocoa butter content.  Generally, one should really rub it in to the entire abdomen, flanks, and breasts twice a day – and rub it into the skin quite aggressively.  Another brand which is gaining popularity recently is Mama Mio, which has two aptly named products to prevent stretch marks:  Boob Tube and Tummy Rub.  Regardless of which product you use, the key is to keep the skin well moisturized – dry skin is more prone to sustaining damage from stretching.  Providing sufficient moisture can increase the elasticity of the skin, so moisturizing creams made from aloe vera, cocoa butter, grape seed, squalene oil, vitamin E, collagen, pro-vitamin B5, germ oil, aroma oil or essential oil can help to prevent the formation of stretch marks. The above ingredients will moisture the skin, increase the elasticity of skin, and protect the skin from damage. However, creams that consist of any exfoliating ingredient like alpha-hydroxy acids AHA , glycolic Acid, Retin-A, vitamin D3 are not suitable to use during pregnancy and breast feeding and should be avoided. The truth is that as long as the product you are using is very lubricating, safe, and feels good to you, then you should go with it!  The thought is to keep your skin as well hydrated as possible as the severe stretching action on your skin can severely damage the underlying collagen and elastin fibers and also cause your skin to dry out significantly.   

 

Once stretch marks are present, they are notoriously hard to treat definitively.  An important point to keep in mind is that stretch marks can never be removed 100%.  For marks that are light in color, there is little one can do to improve them.  Occasionally, the marks can be somewhat red or pink in color.  If such marks do not fade to a lighter shade, they can sometimes be improved with laser treatments using a Candela laser, which is a type of laser used to also treat spider veins or hemangiomas.  Typically laser treatments for stretch marks are easily tolerated, last only several minutes, and require little to no downtime with some possible bruising or temporary darkening of the area being the worst of it.  Each session should cost anywhere from $200 – $750 depending on the doctor you go to and the part of the country you live in.  Laser treatments will rarely work with one treatment, so you may expect to undergo at least 2-3 separate sessions, and some doctors may sell you a package of treatments for a better price.  If there is no improvement after several treatment sessions, then the chances of success are minimal and you may not want to continue with this modality.  There are no proven topical remedies which exist that will “erase” or even significantly improve your stretch marks after pregnancy, despite all the fancy ads out there.  Prevention is key!

 

Of course, depending on where the marks are located, they may be surgically removed if within an area of resection such as during a tummy tuck or breast lift.  If considering any of these kinds of surgeries, you should discuss your expectations concerning eradication of your stretch marks beforehand with Dr. Glatt

Liposuction Northern New Jersey

Morris County Liposuction

MORRISTOWN, NJ – Liposuction is one of the most commonly performed procedures in all of plastic surgery.  It involves the use of blunt tipped probes, called “cannulas”, placed through tiny hidden incisions to remove excess deposits of fat.  Liposuction involves not only fat removal, but also sculpting of the body.  Common areas for liposuction treatment include the abdomen, flanks, hips, thighs, knees, and chest (for men). 

There is much confusion among patients regarding “types” of liposuction which exist.  Many people have heard the terms “Liposcupture”, “Liposelect”, “Laser Lipo”, “Ultrasound assisted liposuction”, and “Tumescent Liposuction”.  The fact is that studies performed and published in the plastic surgery literature show NO BENEFIT or DIFFERENCE between different liposuction modalities!  These terms are heavily marketed by those who have purchased the devices and the techniques have been shown to produce no advantage over more standard techniques.  Many of the claims made of less or no bruising and minimal to no recovery are completely exaggerated and likely to not be true.  It is impossible to undergo surgery which produces results without having to undergo some recovery!

Dr. Glatt perfoms suction-assisted liposuction with tumescence as well as ultrasound-assisted liposuction and is versatile with each.  These are the only two standard liposuction approaches which exist and many of the marketing terms (such as Liposculpture) are actually just one type of ultrasound liposuction machine!

Recovery from liposuction does involve a certain amount of bruising and swelling.  While overall discomfort only lasts for a few short days, the swelling may last for several weeks.  This makes liposuction an ideal treatment during the winter months when bulky clothes can be used to hide treated areas during healing.  Dr. Glatt prefers that his patients wear a compression garment for the weeks following their liposuction, and this helps the swelling and bruising resolve faster.  This garment is provided to you at no additional expense, and is worn underneath your clothing during the healing process. 

Liposuction is also a procedure which is commonly combined with  and easily incorporated into other procedures to further enhance your results, such as a tummy-tuck (abdominoplasty), breast reduction, or breast lift (mastopexy).
